Who it's for

When things are complicated, and the stakes are high

Specialist Support Coordination is funded by the NDIA in a participant's plan when a higher level of skill is needed to manage risk and reduce complexity. We commonly work with situations like these:

Health or mental health crisis

Hospital admissions, discharge planning, or a risk of readmission without the right supports in place.

Contact with the justice system

Police, courts, corrections or community orders — coordinating with justice services around disability supports.

Housing breakdown

Risk of homelessness, unstable or unsafe accommodation, or an urgent need to find a new home.

Many services, no joined-up plan

Several providers and systems involved, but no one holding it all together.

Family or carer arrangements under strain

Informal supports breaking down, or a participant at risk of being left without support.

Language and cultural barriers

Participants and families who need culturally safe support — including in Turkish.

What we do

Reduce the complexity. Keep people safe. Build a plan that holds.

  • Work out what's happening quickly and put immediate supports in place to reduce risk.
  • Coordinate across health, mental health, justice, housing, education and disability services.
  • Develop crisis and risk plans with the participant, their family and their team.
  • Connect participants with the right providers and make sure funding is used well.
  • Build the participant's own skills and confidence to manage their supports.
  • Provide clear progress reports and evidence for NDIA plan reviews.

How to refer

Planners, LACs, hospital and mental health teams, justice and housing workers, plan managers, families and participants can all refer — with the participant's consent.

Helpful to include

  • Participant's name and contact details
  • NDIS number and plan start and end dates
  • Specialist Support Coordination funding in the plan
  • How the plan is managed (NDIA, plan or self-managed)
  • Key risks, current services and urgent issues
  • Confirmation the participant has consented to the referral

Frequently asked questions

What is the difference between Level 2 and Level 3?

Level 2 Support Coordination helps participants understand and use their plan and connect with providers. Level 3 Specialist Support Coordination is for situations that are more complex or high-risk — where several systems are involved and specialist skills are needed to reduce complexity and keep supports working together.

Is it funded by the NDIS?

Yes. Specialist Support Coordination is funded when the NDIA includes it in a participant's plan, within the Support Coordination budget. Not sure if it's in a plan? Send us the plan details and we'll check with you.

Who can make a referral?

Anyone, with the participant's consent — NDIA planners, Local Area Coordinators, hospital and mental health teams, justice and housing workers, plan managers, families, or participants themselves.

How quickly do you respond?

We aim to respond to every referral within one business day.

Which areas do you cover?

We're based in Caroline Springs and support participants across Melbourne's west and Victoria, in person and by phone or video.

Do you support Turkish-speaking participants and families?

Yes — evet! Support is available in English and Turkish.
